PCGH currently operates 7 group homes or supported living programs. These programs encourage clients to be an active part of their treatment team and be involved in planning their services.  Well trained Qualified Professionals and staff in these programs are available to assist with medical needs, securing services, scheduling and attending medical appointments and administering medications.  Well trained direct care staff are also available to work with clients on increasing their skills in the areas of: personal care, daily living, finances, and social and leisure.  Direct Care staff are available on site while clients are at home, however, these homes do not have awake staff during 3rd shift.  PCGH believes that community integration is an essential part of our client’s life and provide opportunities for community activities and involvement.  House meetings that include residents and staff are held monthly to give clients an opportunity to plan activities, voice concerns or make group decisions regarding their home.
