Intermediate Care Facility

Two of the Group Homes operated by PCGH are Intermediate Care Facilities.  These homes are for those with a diagnosis of IDD that need more assistance, supervision or medical care.  An interdisciplinary team, including a Registered Nurse, Registered Dietician, Psychologist, Speech Pathologist, Occupational Therapist, Qualified Professional plus the guardian and individual served are involved in the planning of the Individual Treatment Plan.  Well trained Direct Care staff are available on site 24 hours a day to provide needed care while promoting as much independence as possible.  These facilities do provide awake staffing and supervision throughout the night.  Direct Care staff are also available to work with clients on increasing their skills in the areas of: personal care, daily living, finances, and social and leisure.  Qualified Professionals and staff in these programs are available to assist with medical needs, securing services, scheduling and attending medical appointments and administering medications.  PCGH believes that community integration is an essential part of our client’s life and provide opportunities for community activities and involvement.  House meetings that include residents and staff are held monthly to give clients an opportunity to plan activities, voice concerns or make group decisions regarding their home.
